Supported Living Residential Services for Adults with Developmental Disabilities
Our Adult Supported Living Program provides residential support for adults with intellectual and developmental disabilities. Also referred to as Supervised Living, this is a great solution for adults seeking more independent residency outside of their parents home or a group home. Cozy Horizons will provide the supports that enable the person supported to acquire, retain, or improve skills necessary to reside in their home. This could be an individual’s sole residence or a residence shared with roommates of like interest. Supported Living service is covered in the waivers for Family and Individual Support (FIS) and Community Living (CL).

In-Home Support Service
Our In-home support provides residential support for adults with intellectual and developmental disabilities. This service is a residential service that takes place in the individual's home or family home. This service supplements the primary care provided by the individual, family, or other unpaid caregiver. It is designed to ensure the health, safety, and welfare of the individual. In-Home Support Service is covered in the waivers for Family and Individual Support (FIS) and Community Living (CL).
