In-Home Support Residential Service (Northern Virginia)

Our In-Home Support Service provides residential support for adults with intellectual and developmental disabilities. This service takes place in the individual's home or family home. This service supplements the primary care provided by the individual, family, or other unpaid caregiver. It is designed to ensure the health, safety, and welfare of the individual.

To be eligible for in-home support service:

  • The individual shall require help with adaptive skills necessary to reside successfully in the home and community-based settings.

  • The individual shall be living in his own home or his family home.

This service will enable an individual to acquire, retain, or improve the self-help, socialization, and adaptive skills required for successfully living in their community to include:

  • Skill-building components, along with the provision of supports.

  • Routine and safety supports related to the use of community resources, such as transportation, shopping, dining at restaurants, and participating in social and recreational activities.

  • Supporting the individual in replacing challenging behaviors with positive, accepted behaviors for home and community environments.

  • Monitoring the individual's health and physical condition and providing routine and safety supports with medication or other medical needs.

  • Providing supports with transportation to and from community sites and resources.

  • Providing general supports as needed.

Cozy Horizons will provide supports in a manner which ensures an individual’s rights of privacy, dignity, respect and freedom from coercion and restraint; and which optimizes individual initiative, autonomy, and independence in making life choices.

Cozy Horizons Supported living is licensed through the Virginia Department of Behavioral Health and Development Services (DBHDS). Serving adults 18 years and older.
